Home Health Speech Language Pathologist

CenterWell Home Health seeks a Home Health Speech Language Pathologist to deliver compassionate, in-home assessment and therapy services. You will evaluate communication and swallowing disorders, create evidence-based care plans, and provide patient and caregiver education to enhance safety and independence. Collaborating with an interdisciplinary team, you'll document progress, adjust treatments as needs change, and support quality, patient-centered outcomes in a culture committed to integrity, excellence, and continuous improvement.

Responsibilities

• Perform comprehensive in-home speech, language, and swallowing evaluations for adult patients.
• Develop and implement individualized, evidence-based treatment plans to improve communication and swallowing.
• Educate patients and caregivers on therapeutic exercises, strategies, and safety techniques.
• Collaborate closely with nursing, therapy, and medical staff to coordinate holistic, patient-centered care.
• Document assessments, goals, progress notes, and outcomes accurately in the EMR and per regulatory standards.
• Adjust treatment plans based on patient response and changing clinical needs.
• Promote patient autonomy, safety, and quality of life through functional, goal-driven interventions.
• Participate in case conferences, quality initiatives, and ongoing professional development.
