Home Health Speech Language Pathologist

CenterWell Home Health is seeking a PRN Home Health Speech Language Pathologist to provide in-home evaluation and treatment for adult and senior patients. In this role, you will assess speech, language, cognition, and swallowing disorders; develop and implement individualized care plans; and deliver evidence-based therapies in patients' homes. You will collaborate closely with an interdisciplinary team, educate patients and caregivers, document visits accurately, and support safe, independent living. Responsibilities

• Perform in-home evaluations of speech, language, cognitive, and swallowing disorders for adult and senior patients
• Develop and implement individualized, evidence-based treatment plans
• Provide direct speech-language and swallowing therapy in patients' homes
• Educate patients and caregivers on communication and swallowing strategies and home exercises
• Collaborate with an interdisciplinary home health team to coordinate care
• Document assessments, goals, progress notes, and outcomes accurately and timely in the EMRMonitor patient response, adjust treatment plans, and communicate changes to the care team
• Ensure compliance with clinical, regulatory, and safety standards for home health services
• Participate in case conferences and care planning meetings
• Support patient independence, safety, and quality of life at home
