Nurse Practitioner

CenterWell Home Health seeks a compassionate Nurse Practitioner or Physician Assistant to provide high-quality in-home care. In this role, you will perform comprehensive patient assessments, develop and manage care plans, prescribe and adjust treatments, and coordinate with an interdisciplinary team to optimize patient outcomes. You'll educate patients and families, promote autonomy and safety, and ensure continuity of care in the home setting. Responsibilities

• Conduct comprehensive in-home patient assessments and physical exams
• Develop, implement, and adjust individualized care plans
• Prescribe and manage medications and therapeutic treatments per scope
• Monitor and manage chronic and post-acute conditions in the home
• Interpret diagnostic tests and coordinate necessary follow-up
• Educate patients and families on disease management, safety, and self-care
• Collaborate with nurses, therapists, and physicians to optimize outcomes
• Document care accurately and timely in the electronic medical record
• Ensure compliance with clinical, safety, and regulatory standards
• Participate in quality improvement and professional development activities
