Matrix Medical Network seeks a Nurse Practitioner to deliver in-home health assessments and ongoing clinical care for diverse patient populations. In this field-based role, you will conduct comprehensive evaluations, diagnose and manage acute and chronic conditions, prescribe medications, and develop personalized care plans that support quality, compliance, and cost-effective outcomes. Responsibilities
- Conduct comprehensive in-home health assessments for diverse patient populations.
- Diagnose, treat, and manage acute and chronic conditions following evidence-based guidelines.
- Develop and update individualized care plans in collaboration with patients and caregivers.
- Prescribe and manage medications in accordance with state and federal regulations.
- Document visits accurately and timely in EHR systems to support quality metrics and compliance.
- Collaborate with interdisciplinary teams and payor partners to improve outcomes and reduce costs.
- Educate patients and families on disease management, prevention, and healthy lifestyle choices.
- Participate in quality improvement initiatives and adopt innovative care delivery models.
