Position Summary
Under the general supervision of the Assistant Chief Nursing Officer (ACNO) of Inpatient Services & Staffing Office, the Patient Flow, Admissions & Discharge Registered Nurse (RN) plays a critical role in ensuring safe, efficient, and patient-centered transitions throughout the hospital.
This innovative nursing role is responsible for coordinating and completing patient admissions and discharges on designated inpatient units based on hospital needs. The RN conducts comprehensive admission assessments, medication reconciliation, patient and family education, and other care coordination activities that support seamless patient transitions.
By managing admission and discharge processes, this position reduces the workload of bedside nurses, allowing them to focus on direct patient care while enhancing operational efficiency, patient throughput, and the overall patient experience.
Key Responsibilities
Complete comprehensive admission assessments and documentation.
Perform medication reconciliation and identify potential concerns.
Coordinate and facilitate patient admissions and discharges.
Provide patient and family education related to hospitalization, discharge instructions, and care plans.
Collaborate with physicians, case management, nursing staff, and ancillary departments to ensure smooth patient transitions.
Support patient throughput initiatives and care coordination efforts.
Assist in reducing administrative burden on bedside nurses, allowing greater focus on direct patient care.
Promote safe, efficient, and patient-centered care throughout the continuum of hospitalization.
Schedule: Five (5) - 8hr day/evening shifts with no weekends or holidays
Qualifications
Required
Graduate of an accredited or state-approved Registered Nursing program.
Current Massachusetts Registered Nurse (RN) license.
Minimum of one (1) to two (2) years of clinical nursing experience.
Strong assessment, communication, and organizational skills.
Commitment to providing safe, compassionate, and high-quality patient care.
Preferred
Bachelor of Science in Nursing (BSN) strongly encouraged.

Patient Flow or Throughput Operations
Demonstrated ability to balance exceptional patient care with operational efficiency and patient movement initiatives.
License/Certification
Current Massachusetts Registered Nurse (RN) License required.

Pay Range:
$36.00 - $90.14The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
Anna Jaques Hospital is a 123 bed community hospital serving 17 cities and towns in the Merrimack Valley . The hospital offers a wide range of acute care services to meet the needs of our growing patient population including inpatient and outpatient surgery in fully digitized computerized operating room suites, cardiology including echocardiography and a cardiac cath lab, comprehensive cancer services, orthopedics, nuclear medicine, laboratory, noninvasive vascular lab, joint replacement program and birth center.

The hospitals Non Invasive Vascular laboratory was accredited by the Intersocietal Commission for the Accreditation of Vascular Laboratories (ICAVL).
For the second year in a row, the American Association for Respiratory Care awarded the hospital its Quality Respiratory Care Recognition. The hospital had no ventilator acquired pneumonia cases during the last year.
The Wound Healing & Hyperbaric Center is the only such center to receive full accreditation from the Undersea and Hyperbaric Medical Society (UHMS) in Massachusetts .
