RN Case Manager

Interim HealthCare is seeking an RN Case Manager to coordinate and deliver skilled nursing care to patients in their homes. In this role, you'll perform comprehensive assessments, develop and update individualized care plans, manage medications, and educate patients and families on disease management and self-care. You'll collaborate closely with physicians, therapists, and caregivers to ensure continuity of care, monitor clinical progress, and adjust interventions as needed. Responsibilities

• Perform comprehensive in-home nursing assessments and monitor patient status
• Develop, implement, and update individualized care plans
• Coordinate services with physicians, therapists, and other care team members
• Manage medications and provide disease management education to patients and families
• Document all care, assessments, and communications accurately and timely in the medical record
• Advocate for patient needs and support safe transitions of care
• Communicate regularly with families and caregivers to support care goals
