Job Description
Essentia Health seeks an Occupational Therapist / Physical Therapist to deliver patient-centered rehabilitation services across our integrated health system. In this role, you will assess functional abilities, design individualized treatment plans, and provide evidence-based interventions to improve mobility, independence, and quality of life. You will collaborate closely with an interdisciplinary team, educate patients and families on home programs, and document care in an EMR. Responsibilities
- Evaluate patients and develop individualized occupational and physical therapy treatment plans
- Provide evidence-based therapeutic interventions to improve mobility, function, and independence
- Collaborate with physicians, nurses, and interdisciplinary team members on coordinated care
- Educate patients and families on home exercise programs and adaptive techniques
- Document assessments, goals, and progress according to Essentia Health and regulatory standards
- Participate in quality improvement, continuing education, and clinical best-practice initiatives
- Maintain a safe, patient-centered therapy environment and ensure proper use of equipment
