Job Description
St. David's Medical Center is seeking a Home Health Physical Therapist Assistant to provide compassionate, in-home rehabilitative care in the South Austin area. In this role, you'll implement treatment plans under the direction of a Physical Therapist, assist with therapeutic exercises, gait training, and functional mobility, and ensure safe patient transfers and use of equipment. You'll document patient progress in EMRs, educate patients and families on home exercises and safety, collaborate with an interdisciplinary care team, and uphold high standards of infection control, patient dignity, and clinical excellence.
