VNS Health is seeking a Licensed Social Worker for Hospice Home Care to support patients and families across New York City. In this role, you will complete psychosocial assessments, develop person-centered care plans, and provide counseling, crisis intervention, and end-of-life support in the home. You will collaborate closely with nurses, physicians, chaplains, and caregivers to coordinate community resources and benefits.
