Medication History Pharmacy Technician

LifeBridge Health, Inc. is seeking a Medication History Pharmacy Tech to obtain and document accurate medication histories for patients across our regional health system. In this role, you will interview patients and caregivers, review external records, verify prescriptions with pharmacies, and enter data into the EHR to support safe medication management. You'll collaborate closely with pharmacists, nurses, and providers to resolve discrepancies and support clinical decision-making.
