Crisis Intervention Associate

LifeBridge Health, Inc. seeks a Crisis Intervention Associate to provide rapid assessment, de-escalation, and short-term support for patients experiencing behavioral health or psychosocial crises. In this role, you will conduct risk and safety assessments, coordinate with physicians, nurses, and social workers, and develop crisis and discharge plans that link patients to community and behavioral health resources. You will document in the EHR, participate in interdisciplinary huddles, and support families, helping reduce ER utilization and improving continuity of care for vulnerable populations.
