Social Worker

LifeBridge Health, Inc. seeks a compassionate Social Worker to provide clinical assessments, discharge planning, and resource coordination for diverse patients across our hospitals and practices. In this patient-centered, mission-driven role, you will conduct psychosocial evaluations, develop care plans, connect patients and families with community resources, and support safe transitions of care. You will collaborate closely with interdisciplinary teams, advocate for vulnerable populations, and contribute to quality, equity, and innovation in care while growing your skills within a large regional health system.
