Whole Family Health Center Inc seeks a DME & Home Health Coordinator to manage referrals, authorizations, and delivery of durable medical equipment and home health services. In this patient-centered role, you'll coordinate between providers, vendors, payers, and patients to ensure timely, accurate services, maintain thorough documentation, and verify insurance coverage. You will educate patients and families on equipment and home health processes while supporting quality care, compliance, and access for diverse communities in a compassionate, mission-driven setting.
