Home Health Physical Therapist

Interim HealthCare is seeking a Home Health Physical Therapist to provide PT services to adult and senior patients in their homes across Columbus, IN. In this role, you will perform comprehensive evaluations, develop and implement individualized treatment plans, and focus on improving mobility, strength, balance, and safety to support independence. You will educate patients and caregivers, document care accurately in EMR, and collaborate closely with nurses and other clinicians.
