Hospice RN Case Manager

Interim HealthCare is seeking a compassionate Hospice RN Case Manager to provide expert end-of-life care in patients' homes. In this role, you'll manage a caseload of hospice patients, performing comprehensive assessments, creating and updating individualized care plans, and coordinating services with physicians, social workers, aides, and chaplains. You'll focus on pain and symptom management, patient and family education, and emotional support, ensuring comfort and dignity.
