Job Description
Cambridge Health Alliance seeks an ED RN Case Manager to coordinate safe, timely transitions of care for diverse, underserved patients in a busy emergency department. The RN Case Manager conducts clinical and psychosocial assessments, plans and manages discharges, and links patients to community, behavioral health, and primary care services. This role collaborates closely with physicians, nurses, social workers, and community partners to reduce avoidable admissions, support appropriate utilization, and promote health equity in a mission-driven, teaching-focused environment.
