An organization is seeking a Nurse Practitioner for a Transitional Care Management role in White Plains, New York. This locum opportunity focuses on post-discharge follow-up, care coordination, and supporting safe transitions from hospital to home.
About the Opportunity:
- Shift: Day shift
- Schedule: Monday through Friday
- Hours: Standard business hours
- Setting: Inpatient
- Conduct post-discharge follow-up calls with adult patients.
- Assess patient recovery and identify potential complications or concerns.
- Review and reconcile medications as appropriate.
- Reinforce discharge instructions and provide education to patients and caregivers.
- Coordinate care with physicians, social workers, case managers, and other members of the interdisciplinary team.
Qualifications:
- Active Nurse Practitioner license.
- Experience independently managing adult patients.
- Strong clinical assessment, critical thinking, and communication skills.
- Comfortable conducting patient care primarily via telephone while collaborating with an interdisciplinary team.
Desired Skills:
Background in hospital medicine, primary care, transitional care management, care coordination, internal medicine, or family medicine.
