Job Description
As an RN Case Manager at Ogden Regional Medical Center, you will coordinate comprehensive care for hospitalized patients from admission through discharge. You will assess clinical and psychosocial needs, develop and implement individualized care plans, and ensure safe, timely transitions of care. Working closely with physicians, nursing, social work, and ancillary teams, you will manage utilization, facilitate appropriate services, and support patients and families with education and resources. You will champion quality, safety, and patient advocacy while contributing to a collaborative, learning-focused culture within MountainStar Healthcare.
