Work hours: PRN - Weekends, (Saturdays and Sundays) with PTO Coverage as needed; 8:00AM - 4:30PM
Texas Health Heart & Vascular Hospital is an independently licensed hospital located on the campus of Texas Health Arlington Memorial Hospital. We are a 47-bed facility including a 20-bed Cardiovascular ICU and a 27-bed cardiac telemetry unit.
Texas Health Heart & Vascular Hospital is accredited for Chest Pain through The Joint Commission. We have achieved AHA’s Gold Plus award for three years in a row plus AHA’s Gold Plus award for Heart Failure.
We specialize in CABG and Valve surgeries, thoracotomies, general cardiac including heart failure, cardiomyopathies, AMI, TAVR, Impella, and heart catheterizations including angioplasty, stenting, and electrophysiology studies. We operate in a team-based environment with a cardiac focus. We boast excellent patient and staff satisfaction results have an amazing collaborative culture within all departments.
· Associate degree Nursing Required, or Bachelor’s degree Preferred
· 3 years in an acute care setting, case management or utilization review Required
· One year of hospital case management experience is preferred.
· RN - Registered Nurse Upon Hire Required
· BCCMN - Board Certified Nursing Case Management CPUR, CPHQ, CCS, PCUM Upon Hire Preferred
What You Will Do
· Identify at-risk populations, patients at risk for re-admission. Identify patients who have readmitted or are at high risk for readmission within 30 days with emphasis on HF, COPD, AMI and Pneumonia patients.
· Participates in development of individualized discharge plans to meet patient needs and referral source requirements. Establishes and supports realistic treatment/discharge goals with patients, families, and staff. Applies nursing/clinical knowledge skills and problem-solving techniques to the case management process.
· Manages aspects of discharge planning that require outside services for identified high-risk populations. Maintains active communication with patients, family members, physicians, nursing and other multi-disciplinary team members to develop treatment/discharge plans. Coordinates specific focused discharge interventions and services.
