Social Worker II - Community & Social Services Rutland Regional Medical Center, Vermont's largest community hospital, seeks a Social Worker II to support patients and families within our Community & Social Services Department. The Social Worker II conducts psychosocial assessments, develops care and discharge plans, provides counseling and crisis intervention, and coordinates community resources to address social determinants of health. Working in a highly collaborative, patient-centered environment, this role partners closely with an interdisciplinary team to ensure safe transitions of care and support optimal patient outcomes.
