Mission Health in Asheville, NC seeks a Case Manager Registered Nurse to coordinate safe, high-quality patient care across the continuum. The RN Case Manager conducts comprehensive assessments, develops and updates individualized care plans, facilitates interdisciplinary rounds, and ensures smooth transitions of care and discharge planning. This role reviews clinical documentation for appropriate level of care and resource utilization, provides patient and family education, and collaborates with physicians, therapists, and community agencies. The nurse supports quality, safety, and readmission reduction initiatives while contributing to Mission Health's culture of compassion, teamwork, and continuous improvement.
