Emergency Department Boarder Clinical Pharmacist (7 On/7 Off) - 35 hours, rotating schedule

We are seeking a clinical pharmacist to join our team as an Emergency Department Boarder Clinical Pharmacist. As a Level I Trauma Center and Comprehensive Stroke Center, this position offers the opportunity to practice in a fast-paced, collaborative environment.
The primary focus of this role is to provide comprehensive clinical pharmacy services for patients admitted through the Emergency Department while working closely with the Emergency Department Medication History Technician team to optimize transitions of care and medication management. The pharmacist will also support Emergency Department clinical pharmacists and inpatient teams, with opportunities to cross-cover the ICU service as operational needs arise.
This position also offers opportunities to precept pharmacy students and residents while contributing to quality improvement, interdisciplinary collaboration, and the continued growth of clinical pharmacy services.

Job Description:

1. Practices in accordance with the state and federal laws, rules, and regulations, including but not limited to Massachusetts Board of Registration in Pharmacy, Department of Public Health, the Food and Drug  Administration, and the Drug Enforcement Administration. Maintains certification and credentials in accordance with the Board of Registration and adheres to institutional policies and procedures. 

2. Reviews and verifies medication orders with consideration, including but not limited to the following patient specific parameters: diagnosis, weight/body surface area, age, allergies, organ function, laboratory values, medication duplication, drug interactions. Clarifies orders deemed to be inappropriate, incomplete, or otherwise unclear with the prescriber in a timely manner.

3. Documents and communicates pertinent information and discussion with the healthcare team and pharmacy staff to ensure continuity of care.

4. Provides drug information to physicians, nurses and other health care professionals.  

5. Checks and verifies sterile and non-sterile medications.

6. Supervises the activities of students, technicians, and residents within the pharmacist area of practice. 

7. Promotes a culture of patient safety by submitting  medication related events and near misses.

8. Completes daily pharmacy surveillance tool monitoring, including follow-up and documentation, as well as other daily patient care initiatives and relevant reports.

9. Works collaboratively with all pharmacists, pharmacy technicians, and pharmacy personnel. Upholds a culture of respect and teamwork across the Pharmacy Department.

10. Precepts pharmacy students, resident rotations, and  resident-led quality improvement projects. May serve as Residency Program Coordinator or Residency Program Director

11. Regularly participates in department initiatives including but not limited to supporting formulary stewardship, presenting new medication requests at Pharmacy & Therapeutics Committee, creating and maintaining policies and procedures, conducting assessments of workflows, and/or developing departmental tools and resources.

12. Supports a specific core area of practice through interdisciplinary education, ongoing specialty training and as a resource for Pharmacy staff.

13. Updates clinical guidelines and presents hospital wide educational initiatives.

14. Serves on hospital interdisciplinary committees or workgroups to improve process and quality outcomes.

15. Supports IRB-approved research activities, medication use evaluations, or cost-savings initiatives.

16. Other duties as assigned

Required Qualifications:

1. PGY 1 + 1-3 years OR PYG2 OR 3-5 years of experience in clinical specialty area

1. BPS

 

 

Pay Range:

$63.12 - $84.96

The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law.  Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
