Practice Highlights:
- Predominately outpatient clinical role with meaningful medical director leadership opportunities
- No call, no inpatient responsibilities and no home care
- Small, intentionally designed patient panel that supports high-quality geriatric care

- Operational dyad partnership to advance clinical priorities, care pathways and team development
- 80% clinical (PCP + geri consult clinic) 20% leadership, supported by an operational dyad
Role & Scope of Practice:
- Deliver outpatient geriatric primary care and consult services
- Partner in a dyad structure to translate clinical priorities into operations
- Expand the team by adding 2 APPs with leadership support
- Collaborate with palliative care and specialty partners to strengthen care pathways
