Requirements & Qualifications
- Active Connecticut RN license
- Graduate of an accredited nursing program (BSN preferred)
- Minimum 1 year of hospital or acute care nursing experience
- Valid driver's license and reliable transportation
- CPR Certification
- Strong clinical assessment, documentation, and care coordination skills
- Computer proficiency with electronic medical records
Must Haves
- Independently manage a home health caseload from admission through discharge
- Develop, coordinate, and continuously update individualized care plans with physicians and interdisciplinary teams
- Perform comprehensive in-home assessments and patient education
- Coordinate community resources, specialists, therapy services, and family meetings
- Monitor medication safety, reconciliation, and patient compliance
- Focus on preventing rehospitalizations through proactive care management
- Complete accurate clinical documentation and regulatory compliance requirements
Preferred Background & Skills
- Home Health or Visiting Nurse experience
- OASIS documentation experience
- Medicare/Medicaid regulations knowledge
- Case management experience
- CHAP or Joint Commission experience
- Experience working with interdisciplinary care teams
- Strong patient education and communication skills
