Requirements & Qualifications

  • Active Connecticut RN license
  • Graduate of an accredited nursing program (BSN preferred)
  • Minimum 1 year of hospital or acute care nursing experience
  • Valid driver's license and reliable transportation
  • CPR Certification
  • Strong clinical assessment, documentation, and care coordination skills
  • Computer proficiency with electronic medical records

Must Haves

  • Independently manage a home health caseload from admission through discharge
  • Develop, coordinate, and continuously update individualized care plans with physicians and interdisciplinary teams
  • Perform comprehensive in-home assessments and patient education
  • Coordinate community resources, specialists, therapy services, and family meetings
  • Monitor medication safety, reconciliation, and patient compliance
  • Focus on preventing rehospitalizations through proactive care management
  • Complete accurate clinical documentation and regulatory compliance requirements

Preferred Background & Skills

  • Home Health or Visiting Nurse experience
  • OASIS documentation experience
  • Medicare/Medicaid regulations knowledge
  • Case management experience
  • CHAP or Joint Commission experience
  • Experience working with interdisciplinary care teams
  • Strong patient education and communication skills
