Care Team Collaboration
Partner with the Care Team to deliver safe, effective, and compassionate nursing care
Assess patient needs, create care plans, and assign appropriate team responsibilities
Participate in team meetings and care coordination activities
Build respectful, professional relationships with colleagues
Communicate clearly with team members about patient status and needs
Support team members in complex or challenging situations
Escalate concerns to the Nurse Coordinator when appropriate
Contribute to education and training for team members
Monitor patient satisfaction and support improvement initiatives
Accurately evaluate and document patient outcomes
Administer medications according to established policies and procedures
Direct Patient Care
Provide and document evidence-based nursing care using the nursing process
Deliver care in alignment with hospital, unit, and clinical standards
Administer medications safely and in accordance with all guidelines
Patient & Family Education
Provide timely education based on each patient’s care plan and expected hospital stay
Include family members and caregivers when appropriate
Support the development of educational materials and resources
Document education provided and patient understanding
Ensure patients and caregivers are informed about discharge plans and available community resources
Care Coordination & Length of Stay (LOS) Management
Collaborate with the Care Team to plan care aligned with anticipated length of stay
Work with Nurse Coordinators, case managers, providers, and other departments to support timely care and discharge planning
Coordinate completion of tests, procedures, and consultations
Share clinical updates with appropriate stakeholders
Monitor LOS and support efficient patient progression
Begin discharge planning at admission and include family members throughout the process
Ensure all patients receive clear, written discharge instructions prior to leaving
Required Qualifications
Graduate of an accredited Nursing Program required.
Baccalaureate degree in Nursing preferred.
1-3 years related work experience preferred
License Registered Nurse in the Commonwealth of Massachusetts required.
American Heart Association – Basic Life support Certificate required. ACLS required for some specific clinical units.
Pay Range:
$37.00 - $95.37The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
Anna Jaques Hospital is a 123 bed community hospital serving 17 cities and towns in the Merrimack Valley . The hospital offers a wide range of acute care services to meet the needs of our growing patient population including inpatient and outpatient surgery in fully digitized computerized operating room suites, cardiology including echocardiography and a cardiac cath lab, comprehensive cancer services, orthopedics, nuclear medicine, laboratory, noninvasive vascular lab, joint replacement program and birth center.
Programs include the number one wound center in the nation, a primary stroke service, and Level III Trauma Center . Diagnostic imaging services for patients include MRI, CT, PET, and the PACS digital x-ray system. In addition, we are one of only three healthcare communities to be selected for the pilot Massachusetts e-Health Collaborative. Due to the dedication of our physicians, we are one of the first communities in Massachusetts to implement electronic health records, system-wide, for the safety of our patients.

The hospitals Non Invasive Vascular laboratory was accredited by the Intersocietal Commission for the Accreditation of Vascular Laboratories (ICAVL).
For the second year in a row, the American Association for Respiratory Care awarded the hospital its Quality Respiratory Care Recognition. The hospital had no ventilator acquired pneumonia cases during the last year.
The Wound Healing & Hyperbaric Center is the only such center to receive full accreditation from the Undersea and Hyperbaric Medical Society (UHMS) in Massachusetts .
