A fast-growing healthcare organization is launching a brand-new primary care model and is seeking a high-performing Nurse Practitioner to serve as the first hire in the Norfolk/Chesapeake market. About the RolePosition: Primary Care Nurse Practitioner
Model: New primary care initiative focused on chronic disease management for a complex adult population
Setting:
Primarily in-person clinic-based care across several local sites
Some flexibility for remote charting (half days as needed)
Field-based presence across multiple clinics
This is not a traditional dialysis or specialty NP role it is true primary care delivery within a new, high-visibility model.
Strong support from senior leadership; success of this model is a top strategic priority.

Required Experience:
Minimum 5 years of NP experience in primary care, internal medicine, gerontology, or hospitalist medicine
Proven ability to manage patients with chronic, complex comorbidities
Salary: $130,000 - $145,000/year base
Performance Bonus: Up to 60% of base salary
