Case Mix: General, Ortho, Vascular, Head Spine, Neuro, OB, Regional. No hearts and no peds required.

Support Staff/Supervision: Medical Direction
Credentialing Time Frame: 90 days
Certifications: BC or BE within 5 years of residency. Must have ACLS and PALS
Active OH License: Provider must currently be licensed in OH to be submitted
