What You ll Do
- Provide comprehensive Internal Medicine care in a Value-Based Care Clinic using evidence-based guidelines and value-based care principles
- Deliver population-centered care and help address healthcare disparities through equity-focused clinical practice
- Serve as core faculty for a health equity track, including real-time clinical teaching in primary care, population health, and value-based care
- Supervise and mentor Internal Medicine residents in a clinic-based experiential learning environment
- Participate in journal clubs, lecture series, case conferences, and resident workshops
- Support quality improvement and health equity-focused scholarly activities/projects using outcomes and performance data
- Coordinate team-based care with care managers, pharmacists, social workers, behavioral health specialists, and community partners
What You Need
Required - Current ability to practice Internal Medicine (licensure/certifications not specified in the posting)
- Recent and relevant clinical experience (client note emphasizes updated CVs and qualifying recent experience)
- Demonstrated interest in health equity and value-based care
Preferred - Experience teaching/mentoring residents or involvement in academic or educational activities (e.g., QI, curriculum, conferences)
