PATIENT INFORMATION- Patient Demographics: Adult

- Case Mix: 60% Hematology / 40% Oncology (Solid tumor oncology, Benign hematology, Malignant hematology)

- Support Staff Available: PA/NPs: 3
PRIVILEGES & COMPLIANCE- Temporary Privileges Available: Available for clean files
- Credentialing Timeline: Approximately 90 days
JOB REQUIREMENTS- Board: Board Certified or Board Eligible in Hematology/Oncology
- Licenses: PA license or Universal License (IMLC)
- DEA: DEA required
- Experience: Minimum 1 year experience
SUBMISSION REQUIREMENTS- All required documentation must be submitted no later than 5 business days prior to start
- Provider questionnaire required at time of submission
- NPDB self-query required at time of presentation, within 6 months
- Full disclosure on all malpractice required
- Submissions must clearly state exact dates the provider can work
- Submissions without rates will be rejected immediately
- Required Submission Packet: CV, Coversheet, Questionnaire, NPDB (pulled within the last 6 months)
Duties & Responsibilities- Provide full-time hematology and oncology clinical coverage in a clinic and hospital setting.
- Manage an established patient panel with an average daily volume of 15 patients.
- Provide inpatient and phone call coverage on a 1:2 rotation, including weeknights and two weekends per month.
- Collaborate with a multidisciplinary team including 3 APPs and onsite pharmacy support.
- Participate in disease-specific and general tumor boards.
