Family Practice/Primary Care Nurse Practitioner

Seeking LISW, LPC, or LCSW to provide psychotherapy to adults, adolescents and children with mental heal and substance abuse problems. This included gathering and assessing information, developing treatment plans and utilizing therapeutic interventions such as individual, group, family and marital counseling, and provides referrals to specialists for further testing or treatment when needed. Planning occurs as it relates to the individual, age-specific needs of the patient population.

Work as part of a multi-disciplinary team to provide comprehensive medical care to patients.

Pay: From $31.00 per hour

Minimum Qualifications

Education requirements entail obtaining a Bachelor of Science degree, Masters Degree in Social Work.

• Each provider will be required to have an active license in the state where they practice.
• A copy of the providers CV will be retained in the personnel file.
• Technical Skills: Able to operate a computer, fax machine, printers, scanners, webcam and other office equipment in compliance with our policies and procedures.
• Electronic Medical Records Experience, Centricity/ Athena Practice Experience preferred
• COVID vaccination required upon hire
• Current CPR certification
• Must have excellent oral and written skills and intermediate computer skills
• Must maintain a valid drivers license or dependable transportation
