Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Arizona PT license is required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Outpatient orthopedic experience required. Must be able to perform comprehensive evaluations, develop and implement treatment plans, and document per clinic and payer requirements.
• Experience with outpatient documentation systems including letters of medical necessity, goal writing, and charge entry preferred.
• Ability to supervise and delegate to therapy assistants and to participate in student clinical education is preferred.
• Maintain professional licensure and complete state-mandated continuing education as required.

Assignment Details

• Facility Type: Outpatient clinic.
• Shift Pattern: 5x8 schedule.
• Assignment Duration: 13 week assignment.
• Caseload Focus: Orthopedic outpatient physical therapy evaluations and treatment.
• Responsibilities include comprehensive evaluations, individualized treatment planning, documentation, and participation in performance improvement initiatives.
