Physical Therapist - Travel Contract

Job Description

Looking for a change of scenery and a chance to help others. Provide skilled physical therapy services in a home health setting in Roseburg, OR. Perform patient evaluations and deliver therapeutic interventions per physician orders. Supervise and evaluate physical therapy assistants and home health aides as assigned. Coordinate care, document patient progress, and communicate with physicians and interdisciplinary team members. Must have reliable transportation, current driver’s license, and agency-required liability insurance for home visits.


Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or relevant accreditation body.
  • Current and unencumbered Oregon physical therapy license required prior to start.
  • Current CPR certification.
  • Experience with home health practice, including home visits and home environment evaluation.
  • Familiarity with electronic documentation, email, and basic computer navigation.
  • Reliable transportation, current driver’s license, and agency-required liability insurance.
  • Ability to supervise and evaluate PT Assistants and Home Health Aides in accordance with state regulations.
  • Two to three years of physical therapy experience preferred.


Preferred Qualifications

  • Prior home health therapy experience in Oregon or similar community settings.
  • Experience coordinating care and developing treatment plans in collaboration with physicians.
  • Comfort with telecommunication methods for interim supervision and team communication.


Assignment Details

  • Evenings shift, 5x8 schedule.

Provide skilled physical therapy in patients’ homes to support recovery, functional mobility, and independence. Deliver treatment according to physician plans of care and relevant state regulations. Participate in interdisciplinary care planning, patient and family education, and in-service training. Maintain documentation that reflects patient status and therapy outcomes.
