| Job Summary | The Division of Infectious Diseases is seeking to recruit a Social Worker to expand its growing medical case management team in the Bluegrass Care Clinic. This position will provide medical case management to patients enrolled in the Ryan White Part B program at the Bluegrass Care Clinic. This position will address social needs and work to reduce barriers to care allowing providers and clinical staff to focus on patient care issues. The Social Worker Senior serves as a member of a large multidisciplinary healthcare team dedicated to providing high-quality HIV medical care and a seamless continuum of care to vulnerable people living with HIV in Kentucky. This position is fully funded by Ryan White Part B grant funds issued by the Kentucky Cabinet for Health and Family Services (CHFS). This position is located at Bluegrass Care Clinic, 3101 Beaumont Centre Circle, Lexington, KY. |
