The clinician will plan, implement, and evaluate individualized plans of care to restore or maintain patient function. Responsibilities include assessment, treatment of neuromuscular, neurological, and orthopedic disorders, patient and family education, safe transfer and durable medical equipment instruction, documentation of plans of care and progress notes, and collaboration with the interdisciplinary team. The role requires travel within the local service area and the ability to perform physical tasks such as lifting, bending, and prolonged walking.
