7:00 a.m. – 5:30 p.m. Position Summary We are seeking an experienced and compassionate Registered Nurse (RN) to join our team at an Assisted Living Facility in Beacon, NY. This full-time permanent role is ideal for a Registered Nurse with strong clinical skills looking to make a meaningful impact in a supportive and professional environment. The RN will provide high-quality patient care, collaborate with interdisciplinary teams, and support residents and families while ensuring compliance with clinical and regulatory standards. Responsibilities Schedule / Shift
4 days per week with a weekend rotation
Shift: 7:00 a.m. – 5:30 p.m.
Full-time permanent position
Key Duties
Perform initial and ongoing assessments using tools such as UAS (Uniform Assessment System training provided) or OASIS to evaluate patient and environmental needs
Collaborate with physicians, family members, and supervisors to develop and implement individualized Plans of Care
Deliver skilled nursing services, treatments, and preventive procedures requiring specialized expertise per physician orders
Initiate preventive and rehabilitative nursing procedures to support patient safety and recovery
Monitor and report changes in residents’ physical or emotional conditions while consulting with physicians and supervisors as needed
Accurately document visits, care provided, and patient progress in accordance with organizational policies
Participate in interdisciplinary case conferences and performance improvement initiatives
Educate and counsel residents and caregivers on care plans, treatments, and health-related concerns
Supervise and orient home health aides to ensure quality and continuity of care
Respond to emergencies within the facility with professionalism and urgency
Requirements Education
Required: Graduate of an accredited Nursing program
Licenses & Certifications
Required: Active Registered Nurse (RN) license in New York State
UAS (Uniform Assessment System) experience a plus
Skills
Strong assessment, clinical, and documentation skills
Ability to work effectively within an interdisciplinary healthcare team
Excellent communication and interpersonal skills
Prior experience in home care, assisted living, or adult care facility settings preferred
