NP / Nurse Practitioner / California / Locum or Permanent / NP/PA Opportunities

Job Description:Catapult Healthcare is currently partnered with a company located in Modesto, CA looking to hire Advanced Practitioners onto their team for locum tenens coverage as well as permanent hire. Practitioners will be providing primary care services to the adult population who have been discharged from hospitals, ER's, SNF's, ect and continuing their treatment and follow up care at the patients homes. Please see details below:Details of of the job:-Monday-Friday (8:00am - 5:00pm)-Half day of admin per week-Patient Volume: 4-5 home visits per day-EMR: Allscripts-Procedures: Vitals, BMI, conducting physical exam that includes: urine dipstick and foot exams(as appropriate).-Identify diagnosis to be used in care management and active medical management in the furtherance of treatment.-Formulate a list of current and past medical conditions using clinical knowledge and judgement in addition to the findings of your assessment.-Communicate findings in your assessment that will be used to inform the PCP of potential gaps in care.-Educate members on topics such as disease process, medication, and compliance.Requirements:-BLS certification REQUIRED-Active California Nurse Practitioner/Physician Assistant License-Must have active Nurse Practitioner/Physician Assistant Board Certification-Valid California Drivers License and automobile insurance-NEW GRADUATES WELCOMEBenefits:-Medical and Dental Insurance-401k retirement plan-Short-term and long-term disability-Malpractice insurance covered-Hourly(locums): 60-80/hr Annual(perm): 130,000-150,000/yr - Commensurate to experience-PTO and CME stipend are also includedPlease apply now for more information or to be considered for this position!
