Minimum Requirements

Standard Job Description
A registered professional who provides physical therapy to patients of all age groups. Plans, implements and evaluates patient care plans to restore or maintain patient well being. Plans, organizes and conducts physical therapy treatment program based on the medical referral and the therapist's evaluation. Collaborates with the interdisciplinary team.
Essential Functions
- Perform clinical assessments to identify patient needs and appropriateness for home health physical therapy.
- Document, prepare/review and complete Plans of Care, progress notes, phone orders and other clinical records on an ongoing basis. Revise Plans of Care based on evaluation and ongoing assessment data. Consult with physicians and interdisciplinary team members as needed.
- Plan and provide appropriate treatment for neuromuscular, neurological and orthopedic disorders based on medical referral and therapy assessments.
- Instruct patients and families in transfer techniques and safe use of assistive devices such as walkers, wheelchairs, canes, braces and prosthetic devices. Order and adapt rehabilitative equipment to meet home limitations and patient needs.
- Provide patient and family education tailored to age, condition, culture and learning readiness.
- Supervise and oversee other team members when applicable in performance of patient care consistent with patient needs and goals.
- Apply infection control practices and universal precautions to prevent contamination and transmission of disease.
- Accept clinical assignments consistent with education and competence to care for patients.
- Perform other related duties as assigned or requested.
