Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active WV Physical Therapist license required to start the assignment. Compact PT license accepted.
  • BLS certification from AHA or ARC may be required.
  • Home health experience required. Experience with OASIS documentation and HCHB preferred.
  • Demonstrated skills in exercise programming, rehabilitation, and patient evaluations in homecare settings.
  • Ability to perform comprehensive PT evaluations, develop measurable goals and treatment plans, and document per company, funding source, and regulatory timelines.
  • Experience supervising PT assistants and aides consistent with state practice act preferred.

  • Shift Name: Days.
  • Shift Details: Days 0800-1700. No weekends or on-call.
  • Client requires documentation using OASIS and HCHB.
