Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Georgia Physical Therapist license required prior to the start of the assignment.
• BLS certification from American Heart Association or American Red Cross may be required.
• Home health experience preferred.
• Experience with HCHB (HomeCare HomeBase) preferred.
• Orthopedic patient experience preferred.
• Ability to complete comprehensive PT evaluations, develop measurable goals, and document per company and regulatory requirements.
• Ability to supervise PT Assistants and PT Aides in accordance with state practice act.
• Willingness to cover patients across Henry, Spalding, and Pike counties as assigned.
• Productivity expectation: 30 points (client requirement).

• Shift pattern: Monday through Friday; client will consider either M–F 8:00–17:00 or four 10-hour days.
• License requirements: PT license in state of practice.
• Mileage reimbursement: $0.49 per mile.
