| Job Summary | UK Healthcare is looking for a Family Navigator to serve as a vital connector between patients, caregivers, and the care team, ensuring individuals feel supported, informed, and empowered throughout their healthcare journey. This role blends compassionate, patient-centered support with program development and operational excellence—building and strengthening navigation resources, driving collaboration across teams, and advancing initiatives that improve access, adherence, and overall experience. The Family Navigator plays a key role in removing barriers to care, enhancing communication across the continuum, and using data and partnerships to continuously elevate the impact of the family navigation program.

| Skills / Knowledge / Abilities | Computer, communication, customer service, and data analysis. Knowledge of and experience working with community based resources providing services to the identified patient population. Use of electronic medical record, good communication skills, good problem solving skills. |
