Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active New Hampshire Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience working in a skilled nursing facility or long-term care setting preferred.
  • Experience with common SNF documentation systems and skilled therapy documentation preferred.
  • Ability to provide therapeutic interventions for geriatric patients including mobility training, fall prevention, and restorative programs preferred.

  • Facility Type: Skilled Nursing Facility.
  • Shift Name: Days.
  • Shift Details: Days 0800-1630.
  • Schedule: 5x8.
  • VMS Contract Weeks: 15-week assignment.
