Minimum Requirements
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program, or equivalent degree approved by the APTA.
- Current State License as a Physical Therapist in PA or ability to obtain prior to assignment start.
- Proof of current CPR certification.
- Current driver’s license, reliable transportation, and vehicle insurance.
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting is required.
- Previous home care experience is preferred.
- Ability to perform clinical assessments, develop and revise Plans of Care, document progress notes, and collaborate with interdisciplinary teams as described in the job duties.
- Ability to perform physical tasks required for home health, including heavy lifting, prolonged standing and walking, bending, pushing, and pulling.
- Must read, write, and speak fluent English.
