Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by APTA.
  • Active Illinois Physical Therapy license required to start the assignment.
  • BLS certification from AHA or ARC may be required prior to start.
  • Experience in skilled nursing facility settings preferred.
  • Experience providing outpatient-style and restorative/skilled care in post-acute or long-term care settings preferred.
  • Ability to manage a patient caseload independently and complete documentation in electronic medical records.


Assignment Details

  • Facility Type: Skilled Nursing Facility.
  • Shift Name: Days.
  • Shift Details: Days 08:30–16:30.
  • Schedule: 5x8 (five shifts per week, eight hours per shift).
  • Start: ASAP.
  • Contract Length: 13 weeks.
  • Guaranteed Hours: 35 hours per week.
