Minimum Requirements

  • Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Illinois Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Home Health experience preferred.
  • Experience with OASIS documentation preferred.
  • Experience with HCHB (Homecare HomeBase) electronic records preferred.
  • Minimum education requirement: Bachelor’s Degree.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Schedule: 4x8 (four 8‑hour shifts per week).
  • Weekly hour guarantee: 32 hours.
  • On Call/Weekend Requirement: No.
  • Coverage Areas: Clinton, Marion, Washington, Jefferson, and Hamilton counties; candidate should be prepared for travel between patient homes within these counties.
  • Mileage reimbursement provided between visits from first patient to last patient.
  • Required certification: Illinois Physical Therapy license.
  • Must provide in-home physical therapy evaluations and related skilled services.
