Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Pennsylvania Physical Therapy license required to start the assignment.
• BLS certification from AHA or ARC may be required.
• Experience in home health setting preferred. This includes familiarity with home visit workflows and patient safety in home environments.
• Experience with electronic documentation systems preferred, specifically HCHB and OASIS familiarity is preferred.
• Valid driver’s license and ability to travel within assigned counties is required.
• Minimum education requirement: Bachelor’s Degree.

Assignment Details

• Facility type: Home Health.
• Shift: Days. Schedule: Monday through Friday day shifts.
• Weekly hour guarantee: 32 hours per week.
• Coverage areas: Mifflin County and Juniata County. Candidate must travel between patient homes within coverage area.
• Mileage reimbursement between visits from first patient to last patient is provided.
• On call/weekend requirement: No.
• Required certifications: Active PT license.
• Dress code: blue scrubs.
