Minimum Requirements

  • Master's degree in Physical Therapy required; Bachelor’s or Doctoral acceptable where state regulations allow.
  • Current Physical Therapy license in Pennsylvania required prior to start.
  • Current CPR certification required.
  • Current driver’s license and vehicle insurance, access to a dependable vehicle, or reliable public transportation.
  • Ability to perform evaluations, develop plans of care, and provide home health physical therapy in compliance with state laws and agency policies.
  • Experience documenting in electronic medical records. HCHB (HomeCare HomeBase) experience preferred.
  • Home health experience preferred.


Assignment Details

  • Facility Type: Home Health.
  • Shift Name: Days.
  • Shift Details: Days 0800-1600.
  • Weekly Hours Guarantee: 32 hours per week.
  • Coverage Area: Blair County. Mileage reimbursed between patient visits.
  • Responsibilities include patient evaluation, development and implementation of PT plan of care, progress documentation per agency policy, care coordination with physicians and interdisciplinary team, patient and caregiver education, and working with diverse home environments and patient populations.
  • Dress code: Royal blue scrubs.
  • Education requirement: Master’s degree minimum.
