Job Details
We are seeking an experienced Physical Therapist to provide in-home visits throughout South Hampton area. This is a strong fit for a clinician who values autonomy, flexibility, one-on-one patient care, and meaningful community-based work.
You will evaluate patients in the home, build and direct treatment plans, and help improve mobility, gait, strength, balance, ROM, and functional independence. You will also educate patients and caregivers, coordinate with physicians and the care team, complete documentation, and support discharge planning as needed.
Responsibilities
Perform in-home physical therapy evaluations
Develop and direct individualized treatment plans
Assess gait, strength, mobility, ROM, and rehab potential
Provide patient education and caregiver instruction
Coordinate care with physicians and interdisciplinary team members
Complete documentation, progress summaries, and discharge planning
Evaluate home environment and safety needs
Supervise PTA support when applicable
Requirements
Licensed Physical Therapist in New York
Degree from an accredited Physical Therapy program
Minimum 1 year of PT experience
Home health or community-based experience preferred
Strong communication and documentation skills
Current CPR certification
