Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
  • Active Indiana Physical Therapist license required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience working in Home Health settings is required.
  • Ability to complete comprehensive evaluations, develop measurable treatment plans, and document in required formats in compliance with local, state, and federal regulations.
  • Experience supervising Physical Therapist Assistants and aides in accordance with state practice act.
  • Client-specific requirement: availability to cover visits across Washington, Scott, partial Floyd, and partial Clark counties.
  • Preferred: experience with HCHB and OASIS documentation systems.


Assignment Details

  • Facility type: Home Health.
  • Shift name: Days.
  • Shift pattern: Monday through Friday, 8:00 AM–5:00 PM per client VMS description.
  • On-call requirement: Minimum of one weekend per month on call per client VMS description.
  • Counties covered: Washington, Scott, partial Floyd, and partial Clark.
