Community Care Physicians – The Office of Dr. Lev Barats
Community Care Physicians (CCP), the largest independent multispecialty medical group in the Capital Region, is seeking a Nurse Practitioner or Physician Assistant to join The Office of Dr. Lev Barats, a well-established internal medicine practice focused on delivering comprehensive, patient-centered care for adults.
As an Advanced Practitioner at The Office of Dr. Lev Barats, you will provide adult primary care in a collaborative, team-based environment alongside an experienced physician and Advanced Practice Providers. The practice emphasizes preventive medicine, chronic disease management, and accessible care supported by integrated services and modern patient communication tools.
Conduct comprehensive evaluations for adult patients and manage acute and chronic conditions
Order and interpret diagnostic studies, prescribe medications, and coordinate specialty referrals
Provide preventive care, annual physicals, wellness visits, and patient education
Collaborate closely with physicians, Advanced Practitioners, and support staff to ensure continuity of care
Maintain accurate and timely clinical documentation in accordance with organizational standards
Adult-focused internal medicine practice, caring for patients ages 18+ with an emphasis on preventive care and long-term health management
Comprehensive on-site services, including blood draw, behavioral health support, care management, clinical pharmacy services, and certain diagnostic testing
Telemedicine and secure patient portal, providing flexible access and improved care coordination
Convenient Slingerlands location in the Vista Boulevard area with an established patient base and strong community presence
Stable weekday schedule (Monday–Friday daytime hours) supporting predictable workflow and work–life balance
