Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA or relevant accrediting body.
  • Current, unencumbered Physical Therapist license in the state of assignment (New Hampshire) prior to the start of the assignment.
  • BLS/CPR certification from American Heart Association or American Red Cross.
  • Home health experience preferred, including conducting skilled home visits and evaluating home environments.
  • Experience supervising and delegating to Physical Therapy Assistants and Home Health Aides preferred.
  • Familiarity with documentation systems and basic computer navigation for electronic health records, email, and online training systems.
  • Reliable transportation, valid driver’s license, and agency-required liability insurance for home visits.
  • Two to three years of clinical physical therapy experience preferred.

  • Responsibilities include performing patient evaluations, providing therapeutic services per physician orders, and supervising PTAs and HHAs as assigned.
